TERMS & POLICY: Your reservation will be processed once submitted and a representative will contact you shortly to confirm. No reservations are final until you receive a confirmation number. IMPORTANT: By Submitting this form you authorize United Limousine Service Inc. to charge your credit card on a signature on file basis for your services usage. We require a 5 day advance notice required for any Airport transportation cancellation, 60 Days for any other event transportation. Otherwise you will be charged for the full contracted amount. We offer 30 min. free waiting time for all airport pick-Ups, additional charges apply for times longer than 30 minutes. Deposits are Non-Refundable. Balance must be paid in full 2 weeks prior to event date for all stretch limo reservations. A) Consumption of alcoholic beverages by minors is prohibited in our vehicles. If alcohol is found in our vehicles the job will be terminated immediately with no refund. The driver is not allowed to provide alcoholic beverages. United Limousine Service Inc. and its drivers are not responsible for alcoholic beverages outside our vehicles. B) United Limousine Service Inc. does not assume responsibility for any personal articles left in our vehicles. C) Should the interior passenger compartments and/or equipment of our vehicles be damaged during the contract period, you will be responsible for the cost of damages. D) Should any passenger get sick in the vehicle during the contract period you will be responsible for paying United Limousine Service Inc. a $300.00 cleaning charge. E) NO smoking is allowed, weed, cocaine or any other illegal drugs are not permitted and if any illegal drug/s are found in your possession, smoked, sniffed or consumed the job will be terminated immediately with no refund to you. Also be advised that you are completely responsible for the consequences that may arise with the legal authority personnel/s. G) Be advised that United Limousine Service drivers may take you to a police station if any of the above terms of contract is Breached. United Limousine Service has the right to replace any limo in case of accidents or damage to the limo for a limo that is of equal or greater value. Deposits are non-refundable!
